Activities Assistant

The Wellness Assistant provides individual and group activities appropriate to the needs and interests of each resident to promote well-being and quality of life.  The Wellness Assistant may care for the following age specific population: adults and older adults.

Essential Job Specific Duties/Responsibilities

  • Assists in planning, scheduling and implementing an on-going program of activities designed to meeting, in accordance with the comprehensive assessment, the interests and physical, mental, psychosocial well-being of each resident which includes both individual and group activities.
  • Completes the Resident Activity Assessment in accordance with facility, state and federal guidelines and documents the activity interests and needs of each resident.
  • Assists and implements activities for days, evenings, weekends and holiday.
  • Assists in organizing and scheduling community events related to resident needs and interest.
  • Visits room-bound residents one on one on a daily basis, or as needed.
  • Responsible for maintaining up-to-date knowledge and complying with policies and procedures of Infection Control, OSHA, Safety, State and Federal laws.
  • Interacts effectively within multi-disciplinary teams.

Dining Attendant

The Dining Attendant provides table service to residents, guests and staff.

Essential Job Specific Duties/Responsibilities 

  1. Uses diet cards and menus correctly.
    1. Serves correct diet for each resident served.
    2. Checks diet and menu to insure changes in meal plans are followed.
    3. Reports resident requests for foods not on their meal plan to management.
    4. Responds to requests for special meal needs.
    5. Provides menu items to residents and keeps department informed when menus do not match posted menu.
  1. Performs serving duties in dining rooms or for tray delivery.
    1. Assists in seating arrangement of residents.
    2. Serves beverages at the beginning of the meal.
    3. Serves food as indicated from resident selections.
    4. Insures appearance, quality and temperature are acceptable from steam table to point of service.
    5. Buses and resets tables for next meal.
  1. Insures hot foods remain hot and cold foods remain cold, until the point of service.
    1. Serves only nine meals at a time.
    2. Covers all food when transporting.
    3. Keeps all cold foods iced or refrigerated.
    4. Serves food promptly after leaving steam table.
    5. Takes temperatures using correct procedures and can calibrate a thermometer.

Groundskeeper

 Groundskeepers are required to perform all tasks required to maintain the grounds, gardens, walks and roads in a safe, appealing condition.  All personnel are on-call for snow and ice removal in the winter.

Essential Job Specific Duties/Responsibilities

  • Groundskeepers will operate all equipment necessary to mow, trim, prune, plant and spray grass, trees and shrubbery.
  • Groundskeepers will maintain all roads, walks and outdoor lighting in a safe working condition, free of hazards.
  • Groundskeepers will perform routine maintenance and repairs on all equipment and vehicles, along with appropriate documentation.
  • Groundskeepers will perform a variety of tasks assigned by work orders that relate to the grounds and landscape.

Licensed Practical Nurse - LPN

Under supervision of the Registered Nurse, the Licensed Practical Nurse (LPN) provides resident care in accordance with the Ohio Nurse Practice Act.  The LPN is responsible for utilizing principles of the nursing process in carrying out delegated aspects of care for both residents and families.  The goal of resident care shall be toward restoring and maintaining each resident’s identified capabilities at their maximum mental and physical level. Supports the philosophy, mission and core values of St. Leonard.

  1. Maintains effective communication with residents, families, staff and physicians.
    1. Responsible for daily shift documentation including head to toe review of skilled residents.
    2. Functions in a calm and positive manner in an emergency or crisis situation.
    3. Communicated consistently with the RN Unit Manager; reports pertinent concerns/issues to RN Unit Manager.
    4. Places necessary calls for families and physicians. Demonstrates appropriateness of faxing vs. phone calls to physicians.
    5. Communicates appropriately with physicians; makes rounds with physicians in absence of RN Unit Manager or as delegated by RN Unit Manager. Utilizes physician file appropriately.
    6. Reviews and teaches resident and family about pain management, including pain-rating scales and goal setting; importance of aggressive, preventive pain treatment; analgesic misconceptions; and plan for pain management.
    7. Assists RN in developing, implementing and evaluating teaching plan to meet learning needs of residents and families.

Maintenance Supervisor

The Maintenance Supervisor is responsible for the overall maintenance and operation of the buildings and the appropriate contents. He/She is responsible for inventory control and all required record keeping.

Registered Nurse - RN

    5. Reports to charge nurse if resident eats less than 75% of their meal.
    6. Understands the importance of daily fluid needs.

    6. Reports to charge nurse persistent incontinence and/or inability of resident to provide self-care.

    1. Check residents using incontinence products and changes them regularly.
    2. Provides/assists residents with regular perineal care and with skin care as directed by nurse.
    3. Reports reddened skin or breakdown of skin to nurse promptly, i.e. bony prominence, heels.

    5. Cleans in-room refrigerator and reports non-functioning equipment.

State Tested Nursing Assistant - 2nd Shift

Under the direct supervision of the nurse in charge, the State Tested Nursing Assistant (STNA) performs the basic nursing duties to provide for the comfort, safety and hygiene needs of residents.  The STNA assists residents in maintaining and possibly increasing their level of independence whenever possible.

  1. Provides/assists residents with hygiene needs.

    2. Assists residents with hair care, nail care, oral hygiene (denture and mouth care) and shaving needs.
  1. Assists residents in meeting their hydration and nutritional needs.
    1. Prompts/assists residents, including wheelchair dependent, to the dining room for meals as appropriate.
    2. Serves meal trays to residents in their room promptly and efficiently.
    3. Assists/feeds residents when necessary.
    4. Responsible to see that nourishment/fluids are passed at the appropriate times.
    5. Measures I&O and meal information and records in EMR.
    6. Provides ice water to residents and refills pitchers as needed.
  1. Assists residents to attain or maintain their highest physical, mental, and psychosocial function.
    1. Reports to Charge Nurse upon arrival to determine plan of care for shift and gives report to Charge Nurse at end of shift.
    2. Assists with ambulation and/or transportation needs of resident.
    3. Encourages participation in activities of the facility.
    4. Reports changes in physical or mental functioning to Charge Nurse.
    5. Assists with application of assistive devices and orthopedic appliances as instructed.
    6. Provides ROM exercises as directed.
    7. Follows through on necessary EMR documentation requirements, i.e. bath sheets and ADL’s.
    8. Reports verbal/non-verbal complains of pain/”hurt” to Charge Nurse immediately.

    8. Consistently follows all SAFE program directives, i.e. filling out aide to aide communication sheets, completes safety rounds with oncoming and off-going staff every shift, reviews resident specific fall prevention plans at beginning of each shift and follows plan appropriately and reviews each resident’s care card and follows directed care plan.
    9. Respects residents’ rights and protects residents from complications and injuries.
    10. Follows infection control and prevention procedures.

State Tested Nursing Assistant - Part-Time

Under the direct supervision of the nurse in charge, the State Tested Nursing Assistant (STNA) performs the basic nursing duties to provide for the comfort, safety and hygiene needs of residents.  The STNA assists residents in maintaining and possibly increasing their level of independence whenever possible.

Essential Job Specific Duties/Responsibilities

  • Provides/assists residents with hygiene needs
  • Assists residents in meeting their hydration and nutritional needs.
  • Assists residents to attain or maintain their highest physical, mental, and psychosocial function.
  • Provides/assists with maintenance of bowel and bladder functioning.
  • Provides/assists in maintaining skin integrity.
  • Performs designated nursing procedures.
  • Assists in providing a clean and safe environment.
